The state of Kansas has recently updated their immunizations requirements for students attending Kansas schools for the 2010-2011 school year. These new requirements are available on the Kansas Department of Health website.








If your child has been diagnosed with fifth disease, or if you are wondering what the symptoms might be, information is available at the following CDC website.




USD 464 is participating in a medical insurance policy with Student Assurance Services (SAS) which will provide benefits for accidental bodily injury to students, incurred while attending regular school sessions, participating in or attending school-sponsored and supervised extracurricular activities, or traveling directly to and from school for regular school sessions; and while traveling to and from school sponsored and supervised extracurricular activities in school provided transportation. This does not cover participation in any interscholastic sports. 

If your student is injured during one of the above situations, the school nurse or athletic trainer must file a report as soon as possible. Please make sure that they are aware of the injury so a claim form may be filed. A claim form and additional information is available on the SAS website.

If your student participates in interscholastic athletics, coverage does apply under the same policy, but with a $3,500 deductible. Additional insurance is available to cover this deductible. Prices vary by sport and age. For information call the district office at 913-845-2153 and speak with Mrs. Carlisle.





In order to participate in any inter-scholastic activities at the middle school or high school level, a student must show that they are healthy enough to compete and do not have any underlying physical issues that would endanger their health. To that end, the Kansas State High School Activities Association (KSHSAA) requires a Pre-Participation Physical Evaluation Form to be completed each year by each participating student, their parent/guardian, and a qualified physician. Failure to complete the form and turn it in to the school office will render the student ineligible to compete or participate in any way in school athletics.
